Camp Phoenix 2026 Online Application Form

Please complete a separate application form for each camper wishing to attend Camp Phoenix 2026

Applications must be completed no later than July 25, 2026 (any applications after the deadline will be waitlisted)

Applications will be considered on a first-come-first-serve basis.
